Quick Take

  • Your plan and your doctor go together. Open Enrollment (October 15 – December 7, 2026) is the best time to make sure your 2027 plan covers the primary care doctor you want.

  • If your doctor leaves your plan's network mid-year, you generally can't switch plans just for that reason.

  • In California Medicare Advantage HMOs, you often need to check both the doctor and the medical group.

  • A good Medicare primary care doctor should offer Annual Wellness Visits, medication reviews, chronic-condition care, and easy ways to be seen.

  • Call the doctor's office before you enroll. Don't rely only on online directories.

1) Why Open Enrollment Is the Time to Think About Your Doctor

Most people think of Open Enrollment as choosing a plan. But for many seniors, the more important choice is the doctor.

Your primary care doctor:

  • Manages your chronic conditions (diabetes, blood pressure, heart and lung disease)

  • Keeps your medication list accurate and safe

  • Sends referrals and handles prior authorizations

  • Coordinates care after hospital or ER visits

Many Fresno seniors are looking for a new primary care doctor this year because:

  • Their doctor retired or stopped taking their plan

  • Their plan is leaving the area for 2027

  • They have trouble getting appointments

  • They want a doctor who takes more time with them

The San Joaquin Valley has fewer primary care doctors per person than many parts of California. Planning ahead matters.

2) Why You Can't Wait Until Mid-Year

For 2027, Medicare considered a rule that would let patients switch plans mid-year when their doctor leaves the plan's network. That rule was not approved.

That means:

  • If your doctor isn't in your 2027 plan, you may be stuck until the next enrollment period.

  • Medicare Advantage members get one change from January 1 to March 31, 2027, but after that, options are limited.

  • Fresno networks can change. In 2026, contract disputes between Community Health System and some health plans temporarily left patients out of network.

Bottom line: Choose your doctor and your plan together, before December 7.

3) How to Check if a Doctor Takes Your Plan

1. Write down the exact plan name (for example, the full HMO or PPO name, not just the insurance company).

2. Check the plan's 2027 provider directory, either online or by calling the plan.

3. Call the doctor's office and ask:

  • "Do you accept [plan name] for 2027?"

  • "Which medical group do I need to choose?" (Many California Medicare Advantage HMOs work through a medical group.)

  • "Are you accepting new Medicare patients?"

4. If you have Original Medicare, ask: "Do you accept Medicare assignment?"

5. Write down who you spoke with and the date.

4) Questions to Ask Before Choosing a Primary Care Doctor

  • Are you accepting new Medicare patients, and how soon can I be seen?

  • Do you accept my plan and medical group for 2027?

  • Do you offer same-day or next-day appointments when I'm sick?

  • Do you offer telehealth visits?

  • Do you provide Annual Wellness Visits?

  • Will you review all my medications, including ones from specialists?

  • How do you handle referrals and prior authorizations?

  • Which hospitals and specialists do you usually work with?

  • Do you have labs or tests on-site?

  • Can a family member be involved in my care and appointments?

5) What a Good Medicare Primary Care Practice Should Offer

  • Annual Wellness Visits (covered by Medicare with no cost to you when eligible) to plan screenings and prevention

  • Medication reviews, especially if you take 5 or more medicines

  • Chronic-condition care for diabetes, high blood pressure, heart disease, COPD and kidney disease

  • Timely access: same-day visits for urgent problems so you can avoid the ER when it's safe

  • Telehealth. Medicare telehealth flexibilities, including visits from home, are extended through December 31, 2027.

  • Help with paperwork: prior authorizations, referrals and coverage changes

  • Follow-up after hospital or ER visits

6) How to Switch Doctors Smoothly

1. Pick your doctor and plan together before December 7.

2. Request your medical records from your previous doctor (or sign a release so the new office can request them).

3. Bring an up-to-date medication list, or bring all your pill bottles to your first visit.

4. Ask about existing referrals and prior authorizations. Many insurers have pledged to honor existing approvals for 90 days after a plan switch, but confirm with your new plan.

5. Update your Medicare Advantage plan if it requires you to name a primary care doctor.

6. Book your first visit early in January, especially if you need refills.

7) Frequently Asked Questions

Can I change my primary care doctor without changing my plan?

Often yes. Many Medicare Advantage plans let you change your primary care doctor within the network, sometimes starting the next month. Call your plan.

You generally can't switch plans just for that reason. You can choose another in-network doctor, or use the next enrollment period.

You can see any doctor who accepts Medicare. You should still confirm the office is accepting new patients.

A yearly prevention-focused visit to review your health, medications, screenings and goals. It's covered by Medicare with no cost to you when eligible.
