Starting 2025, your Part D out‑of‑pocket drug costs are capped at $2,000 for covered medications. Hit the cap, and you owe $0 for covered Part D drugs for the rest of that calendar year. In 2026, the threshold is $2,100 (indexed annually). CMS National Training Program+1

What the cap does
Predictability: After your cumulative Part D cost‑sharing reaches $2,000 (2025), you pay $0 for covered Part D meds for the rest of the year. CMS National Training Program
Applies across phases: Deductibles and coinsurance you pay for covered Part D drugs all count toward the cap.
What the cap doesn’t do
It doesn’t reduce premiums or copays before you hit the cap.
It doesn’t apply to non‑covered drugs or Part B drugs (e.g., some infusions in clinic).
It resets each January 1. (Annual cap.) CMS National Training Program
Example (simple math)
You pay $500 in the first quarter, $1,200 by summer, and another $300 in the fall = $2,000. After that, $0 for covered Part D drugs through Dec 31. (Your plan’s tracking handles this automatically.) CMS National Training Program
Pairing the cap with the Medicare Prescription Payment Plan (MPPP)
If your costs spike early in the year, MPPP lets your plan bill you monthly instead of paying at the pharmacy counter. Helpful for cash flow, but not a discount. You still pay up to the cap. Medicare+1
Who benefits most?
People on high‑cost meds
Those with a few expensive brand drugs
Anyone wanting stable monthly budgeting via MPPP (optional)
